Free iep meeting notes template
An IEP meeting moves fast and covers a lot. These notes help parents and school staff record what the team agreed: the child's present levels, the goals, the services and accommodations, any concerns raised and who does what next. They sit alongside the official IEP document, not instead of it.
Paste it into Google Docs, Word, Notion or an email. Replace the [brackets].

How to use this template
- Ask for the draft IEP and any evaluation reports a few days before the meeting, so you can prepare questions.
- Write goals and services exactly as they were agreed: the minutes per week, the setting and the start date.
- Note anything the team didn't agree on, and what happens next, rather than leaving it out.
- Send your notes to the team the same day and ask them to point out anything you got wrong.
- Check your district's policy before recording an IEP meeting, and tell everyone in the room first.

Who should attend an IEP meeting?
In the US, the IEP team includes the parents, at least one general education teacher if the child is or may be in general education, at least one special education teacher, a school or district representative, someone who can explain evaluation results and, when appropriate, the student. Others with knowledge of the child can attend too.
What should parents write down at an IEP meeting?
Who attended, the present levels, each goal and how progress will be measured, the services with minutes and setting, the accommodations, your concerns, anything not agreed and the next steps with dates.
Can I record an IEP meeting?
It depends on your state and school district, which set their own policies. Ask the school before the meeting, and tell everyone present before you start recording.
How often is an IEP reviewed?
At least once a year, and the team can meet sooner to review or change it.
Is this iep meeting notes template free?
Yes. Copy it into Google Docs, Word, Notion or an email and use it as often as you like. No sign-up needed.
Can AI fill in the iep meeting notes for me?
Yes. Acta records the meeting on Zoom, Google Meet or Microsoft Teams, with or without a bot, or in person with the mobile app, and writes the notes, decisions and action items with owners afterwards. Anything the meeting didn't cover is left out, not made up.
